normally, if an inmate is
likely to be discharged/
transferred
their property is retrieved,
boxed, secured w/ property
form by R&D staff, all items
accounted for + inventoried
All MCC staff identify themselves & must sign logbook
to enter SHU if not assigned
there. Staff (CO) will
physically escort them or
will pick up property @ unit door
{Page 42 - EFTA00130190}
not uncommon for Reyes to be
gone for extended period
Capt [REDACTED] was not
notified that his belongings
were removed
C.O.’s would have known
Reyes belongings were
removed
C.O. should have notified
LT who would have
briefed up notified capt [REDACTED]
If he was notified capt
[REDACTED] would have likely
put Epstein on psych obs
[REDACTED] is not aware of

Sat morning around 7 AM,

where is his cell mate?
He didn’t have a cellmate
He was assigned a cellmate
b/c I was involved in

Fri @ 4pm-10pm - ops Lt [REDACTED]
assigned responsible for SHU and all of building
Fri 10pm -

documented in TRUSCOPE (elec. logbook)
C.O. log in &

before button is clicked
unfortunately, sometimes officers
don’t do 30 min rounds or
exceed 30 min threshold
It is documented what location/terminal
rounds are logged from
2 terminals in SHU that [REDACTED]
is aware of
only way to determine if roun

Mr. [REDACTED] camera tech,

w/o supervisor present
Epstein was placed on floor

Some of his staff were under

from SHU (not true)
3:00/5:00 count - physically
count person in each cell
informed AW [REDACTED]
what Noel & Thomas admitted
to Lt. [REDACTED]
no entries of counts in TRUSCOPE
for the entire night
could be hardcopies if tech is down
{Page 48 - EFTA00130196}
no electronic records of counts the entire night
all phone calls in SHU are
monitored, limited phone calls
calls should be recorded
never heard about complaints/concerns over
phone calls
aware that Epstein
made a call @ 7pm
on Fri night 8/9
(he was told this on Sat 8/10)
uncommon to make an unrecorded
phone call in SHU
would advise against it b/c it
is not surveilled
can make a recorded call in
Lt’s office, documented
in a monitored logbook.
Capt [REDACTED] is not briefed on
phone calls in the SHU generally
{Page 49 - EFTA00130197}
in SHU, C.O.’s are not permitted
to give inmates phone calls
but unit team member or
chaplain can take inmate to
Lt’s office to make call
did best to supervise, safeguard, + ensure
protection of inmate effectively,
wholeheartedly
staff aware of seriousness
