went to Shu 7/16, cell 217 L tier, cellmate was a [illegible] latin king
things got out of the box ~ 2 days later, then had Gilbert Amant[?]
for about a week, then when he left, they brought Chuckie (Y6[?])
then chuckie was moved to go w/ cross, put Reyes w/ Luigi (214)

then co told Reyes to pack up + moved him - "Fonzi" was in
220 and moved him to 6 tier (single man cells)
Lt [REDACTED] big husky guy - Lt. for SHU went in cell + said you don't give us a hard time, I got a
cool bunkie, he's a old guy. he's not going to bother you, but
don't bother him - Reyes says I have a bottom bunk passs,
[REDACTED] says don't worry he sleeps on the floor - CO
disciplined Reyes in the past

in 220, Epstein walked in - ER told [REDACTED] in Spanish you jammed me
up [REDACTED], he just tried to hang himself I know that he
just came from suicide watch, [REDACTED] says he's ok, don't
worry about it, keep an eye on him, it's going to be alright
Reyes said what am I SPA - suicide prevention advocate
(Rikers term but know that
they have them at MCC but
not in cell w/ you - have
seen suicide watch area
and SPA sit IPO door)
[REDACTED] laughed + said you're
going to be alright
he brushed him off

heard that Epstein tried to hang himself w/ killer cop[?]
heard that they took him out to suicide watch
heard from other inmates in SHU screaming

when JE came in, they brought 2 mattresses in
hispanic officer whose last name starts w/ [REDACTED], jelled up hair w/
fade (name in camo) + [REDACTED] - grey salt pepper hair retiring
in a few months - the ones who put [REDACTED] don't wear
nametags - hispanic

{Page 20 - EFTA00132227}
- ER thought they were going to put on top rack but they
put on floor
- JE put mattresses on the right - when door is closed
you can't see the space. JE is towards the right w/ feet
under the bunk w/ head towards the door.

- told JE you high profile, please don't do nothing while I'm
in here, I have a chance to go home soon. I know you must
have your reasons to do what you want to do. I don't care
what you did. That's all I ask. If I can help you, let me know

- told JE don't hang yourself up in my cell. Don't try to kill
yourself in this cell. I don't want to wake up + find you dead.
-he said don't worry I'm never going to cause you trouble
JE arrived after legal visit

- ER said I'm smoking a deuce, if we're bunkies, you can't
go tell on me and jam me up either, he said don't worry

prior employment - juvenile services
BOP - 2009 in Virginia started
March 2011 - MCC (transferred)

Bid on post every 3 months
Currently receiving & discharge (R&D) since 2014 (main post)

Day on incident, was on “Control 1” (overtime)

Has worked internal, OP (outside) (SHU) special housing unit

whenever overtime is available, [REDACTED] usually works it
Internal is responsible for counting, & responding to emergencies
and anything the LT wants; counting is also annotated by
“count slips”
Internal helps with all the counts, sometimes its 2, sometimes
internal counts all except for 10 south & 9 south
Can start the count from the top or bottom

SHU morning shift watch, print off roster, and make rounds on the unit
Make rounds every 30 minutes
Counts at 12:00, 3:00 & 5:00
“I count every single time for the 30 minute rounds.”

{Page 139 - EFTA00132346}
2
Counting rounds are to be infrequent (ie: 30-40 min) rounds
so inmates cannot anticipate the exact time the CO
appears on the range.

Calling the control for opening the door, or a “watch call”
watch call - every 30 minutes

Whenever the call is complete, CO calls the count in to control
Whenever the count is initiated CO calls it in to control as well

Control - monitor the activity, answer calls, answer radio, open door.
“No one is really moving anywhere within the institution”

“E1” count sheet printed off from “Sentry”
control validates the count number, marks with an “x.”
check for every unit, plus all accurate count slips.
Staples all “E1’s” so its documented then considered
a good count

(shift)* 12-8 Aug 10th [REDACTED]
Lieutenant took the 12 o’clock count that day
[REDACTED]
Cannot physically see what the CO’s are doing
on the range.

Recalls SHU called in their count the day of the incident
and that it was a good count.

Does not recall who called from the SHU, the count
was “72.”

There are folders for the day shift, that compile
all the count verification time/count sheets for the
specific day

5:00 AM count; SHU called in the count

Believes Nowell called for the “46” door to go
use the bathroom or (10 south). ppl use the stairs
there is no elevator from (9 south to 10 south).

{Page 141 - EFTA00132348}

All doors that are opened must be called to “control”
to open or “pop” the door

There are 2 doors; external & internal doors
Control pops the external door, internal officer can pop
the internal door

“Review of call times” [REDACTED] Lieutenant’s extension
(5:09 AM)
Remember seeing Lieutenant going to SHU around the
3 o’clock count

Does not know if there is a record of doors that
have been opened by “control”

6:33 A.M. HIT body alarm (lieutenant)
body alarm check, medical emergency

Control called 911 (control has direct line to EMS)
right after body alarm medical emergency at 6:33 A.M.

Did not know what was happening but called EMS
for immediate response

Recalls allowing/operating elevators up & down
